The SOCMAR Student Agreement Form is in Excel and can be downloaded for use as a Student Agreement template using the link below. It contains formulas that automatically total each of the columns as you fill them out.

Please retain your downloaded copy for future reference. (You may need to move the Excel file to a different folder/directory before quitting your Web browser to prevent from deleting the file.)
